CES offers a choice of 2 PPO Plans: a 2000/4000 option and a 3000/6000 option. Review the Schedule of Benefits for each PPO Plan available below. If you have further questions, call HPHC (800) 708-4414.

To enroll for the first time: you can enroll either at hire or during Open Enrollment

A "Must Have" for PPO Plan Members!

PPO members are able to participate in a Health Savings Account, which can be used for many medical expenses. HSA funds are yours to keep - there is no "use it or lose it" rule - the money is yours and you can take it with you should you leave CES.

Consider a Health Savings Account (HSA) to pay for deductible expenses. Below is information about the HSA and how it can help you pay for deductible expenses.

To set up your HSA, you will need to do the following:

  1. Open an account online. Click below for helpful instructions.
  2. Complete the Payroll Deduction Form and return to the CES Human Resources Department
