Algebra Student Registration

• they have previously failed Algebra A or B
• they need to make up part of an Algebra course to obtain credit for the course
• they would like to participate in accelerated Algebra study
• they have have an active email account (see below)  

 
  Those who qualify must complete the 4 steps below:

  Every student must complete the online registration form: 

• Click here to complete the online registration form. Please fill this form out completely.
• Students who do not have an email account will need to create one now. We suggest using a Google Mail, Yahoo, Comcast, or other email account. This email must be accessible within the student's school.

  Every student must ALSO submit a paper form SIGNED
  by the student and a parent/guardian:

• Click here to download the paper agreement form, which must be completed and signed by the student and parent/guardian

• Return the signed and completed agreement form to the attention of Kristin Kicza via fax (413.586.2798) or mail (Collaborative for Educational Services, 97 Hawley Street, Northampton, MA 01060). • Guidance Counselors (or a school representative) may help register a student using the online Registration Form.
• The signed paper agreement form must be completed and submitted BEFORE the student enters the course.
  
  

 
   Students must open, read, and agree to
  the CES Acceptable Use Policy. 

• Open and read, and acknowledge your agreement to adhere to the Internet Acceptable Use Policy by completing this form. When you click on the link above, the policy will open in a new window with instructions.

 
   Once the student has been registered, a confirming email
  will be sent to the student's email address. 

• Check your email for a confirmation notice with log-in information.
